Here it is. One page, twelve items, with a blank next to each one so you can write down where yours lives.

A word of advice before you dive in. Do not try to do all of it this weekend. Pick number one, the birth certificates. Find them, put them in one folder, and stop there. You will have done more than most families ever do.

When you are done, tell one other person where that folder lives. Being organized is a private act. Being findable is a favor you do for someone else.
